Job Title: Quality Measures Specialist

Pay: $22/hr 

Job Type: Full-Time (40 hrs/week)
Schedule: Monday – Friday 8AM-5PM

Job Description:

We are seeking a dedicated and detail-oriented Quality Measures Specialist to join our team. The Quality Measures Specialist will play a key role in driving quality improvement efforts across the organization by working closely with physicians, clinical staff, and patients. This individual will manage and support various quality-related projects and initiatives aimed at enhancing patient care.

Key Responsibilities:

  • Develop, coordinate, and implement direct patient outreach campaigns, including contacting patients by phone to discuss aspects of their care.
  • Utilize call scripts to inquire about patients’ preferences and needs regarding prescription medications.
  • Encourage patients to complete preventive screenings by conducting outreach calls.
  • Contact doctor's offices to relay important information, including patient outreach results, project follow-ups, and other relevant details.
  • Document all patient outreach results and other related activities accurately and efficiently.
  • Perform reconciliation of closed quality measures between health plan data and IPA data aggregator.
  • Create non-standard files for health plan data submission and quality measure closure.
  • Fax, mail, or distribute reports and materials to physician offices.
  • Maintain compliance with HIPAA standards to ensure patient privacy is upheld.
  • Collaborate with the Provider Incentives Department on quality-related projects and initiatives.
  • Use internet-based platforms to identify open care gaps and generate reports.
  • Participate in meetings (both virtual and in-person) with health plans, medical groups, and other organizations.
  • Maintain a professional demeanor when communicating with patients and healthcare providers.

Requirements/Qualifications:

Required:

  • High School Diploma, GED, or equivalent level of education.

Preferred:

  • Previous experience in a call center, customer service role, or similar (1+ years).
  • Bilingual in Spanish.
  • Proficiency in Microsoft Office (Word, Excel).
  • Strong communication skills, both written and verbal.
  • Highly motivated with a strong desire for professional advancement.
