We are seeking a Clinical Manager for an immediate start opportunity in Somerville, NJ. A well established home health care service is seeking an experienced, and qualified clinical manager for their facilities. Monday - Friday:
8:30am - 5:00pm
Essential Responsibilities:
- Participates in the initiation, development, and implementation of policies, programs and procedures.
- Interviews, orients, supervises, and instructs nursing and therapy personnel according to home health agency requirements.
- Assign cases to appropriate personnel in planning for the management of the patient care plan and coordination of needed services.
- Ensures development, implementation, and updates of the individualized plan of care.
- Review patient clinical records to ensure documentation complies according to the standards set by agency policy and procedure.
- Monitors patient plan of care, utilization of resources, and reimbursement for each patient under the team.
- Participates in patient/family conferences to assist in the resolution of patient care problems.
- Monitors and evaluates intake/referral functions to ensure a smooth and thorough mechanism for entry into our system. Communicates barriers to the Director of Patient Services.
- Provides a professional environment conducive to problem-solving and promotion of effective
interpersonal relationship.
- Assumes a leadership role in representing the agency to other community groups.
- Participates in the development of policies and procedures required by federal, state and/or local regulations pertaining to the provision of patient care services.
- Ensures staffing patterns and organization of caseloads to meet patient’s needs.
- Provides coordination and direction to all agency services, i.e., nursing, therapists, social worker, nutritionist, and home health aides.
- Serves as liaison to Utilization Review and Professional Advisory Committees.
- Participate in activities that enhance professional development and knowledge.
- Ensures staff comply with federal, and state regulatory requirements concerning documentation and timeliness of submitting documentation.
- Contributes to the overall ability of the agency to meet the patient’s needs and the mission of the agency.
- Participates in the clinical placement of professional nursing students within the agency.
- Evaluate and supervise staff’s job performance. Documents staff performances ongoing.
- Participates in the identification of areas within the agency requiring changes and develops plans for action regarding these.
- May substitute for other nursing personnel as necessary.
- Covers weekend and after-hours schedule on a rotation basis.
Required Skills and Competencies:
- Bachelor''s degree in Nursing
- Valid NJ RN License
- Valid Driver''s License
- 2-3 years combined Public Health Nursing and progressive professional responsibilities in Public Health Nursing
- Ability to stand, walk, sit, climb stairs, reaching, kneeling, bending, squatting, lifting, carrying/pulling pushing up to 25 lbs.
- Use of telephone, photocopy machine, fax machine, computer, all DME and nursing supplies, e.g., phlebotomy, catheters, wheelchairs, etc.
