Job Full Description

Position Summary:
A Case Aide provides administrative and support services to case managers, social workers, and other professionals in social service or healthcare settings. They assist with client interactions, maintain documentation, and help ensure clients receive the resources and services they need.


Key Responsibilities:

  1. Administrative Support
    • Assist case managers in organizing and maintaining client files and records.
    • Schedule appointments, meetings, and follow-ups for clients and staff.
    • Prepare and distribute reports, forms, and correspondence as required.
  2. Client Assistance
    • Serve as the first point of contact for clients, answering inquiries and providing basic guidance on available services.
    • Assist clients in completing applications or forms for programs and services.
    • Provide transportation or accompany clients to appointments when necessary.
  3. Resource Coordination
    • Help clients connect with community resources such as housing, food assistance, and medical care.
    • Collaborate with local organizations and agencies to coordinate services.
  4. Documentation and Compliance
    • Ensure accurate and timely data entry into case management systems.
    • Maintain confidentiality and comply with all legal and organizational standards for record-keeping.
  5. General Support
    • Monitor and follow up on client progress under the supervision of case managers.
    • Participate in team meetings and provide updates on assigned tasks.
    • Assist with special projects or events to support the organization's mission.

Qualifications:

  • Education: High school diploma or equivalent (Associate's or Bachelor's degree in Social Work or related field preferred).
  • Experience: Prior experience in social services, healthcare, or a similar field is beneficial.
  • Skills:
    • Excellent communication and interpersonal skills.
    • Strong organizational and time management abilities.
    • Proficiency in Microsoft Office and case management software.
    • Ability to work effectively in diverse and sensitive environments.

Physical Requirements:

  • Ability to travel locally to meet clients or attend appointments.
  • Occasionally lift and carry materials or equipment weighing up to 25 pounds.

Work Environment:

  • Office-based with some fieldwork, depending on client and program needs.
