Job Summary
The Case Manager plays a pivotal role in supporting individuals and families by providing guidance, resources, and advocacy to help them achieve their personal goals. This position requires a compassionate and organized individual who can assess client needs, develop tailored service plans, and facilitate access to necessary services. The Case Manager will work collaboratively with clients, community organizations, and service providers to ensure comprehensive care and support.

Responsibilities

  • Conduct assessments to identify client needs and develop individualized service plans.
  • Provide ongoing support and case management services to clients, ensuring they have access to necessary resources.
  • Facilitate communication between clients and various service providers, including healthcare, social services, and educational institutions.
  • Monitor client progress towards goals and adjust service plans as needed.
  • Advocate for clients in accessing benefits such as Section 8 housing assistance or Medicare services.
  • Utilize motivational interviewing techniques to empower clients in their decision-making processes.
  • Mentor and supervise interns or junior staff members when applicable.
  • Maintain accurate documentation of client interactions and progress in compliance with organizational policies.

Skills

  • Strong understanding of child welfare practices and principles.
  • Experience in social work or related fields is highly desirable.
  • Knowledge of workforce development strategies to assist clients in gaining employment.
  • Familiarity with behavioral health issues and resources available for support.
  • Effective mentoring and supervising skills for guiding less experienced staff or volunteers.
  • Ability to engage in motivational interviewing to foster client empowerment.
  • Experience with childcare practices is a plus.
  • Excellent communication skills, both verbal and written, for effective client interaction.
  • Strong organizational skills to manage multiple cases efficiently.
