Position Summary:
The Assistant Case Manager will provide support to the Case Management team by assisting in the delivery of services to individuals and families who are experiencing homelessness, poverty, or crisis situations. This role involves assessing clients' needs, helping to develop service plans, and supporting clients as they work towards achieving their personal goals. The Assistant Case Manager will work within company’s mission to meet the physical, emotional, and spiritual needs of clients.
Key Responsibilities:
- Client Intake and Assessment: Assist with intake procedures, conducting assessments, and gathering client information to determine eligibility for services.
- Resource Coordination: Connect clients with appropriate resources such as housing, employment, healthcare, food, and legal services.
- Follow-up and Monitoring: Support case managers in tracking client progress, providing ongoing follow-up, and adjusting service plans as necessary.
- Documentation: Maintain accurate and up-to-date client records, case notes, and progress reports in compliance with program standards and confidentiality regulations.
- Crisis Intervention: Assist clients in addressing urgent needs and help manage crisis situations as they arise.
- Collaboration: Work closely with other team members, agencies, and community partners to ensure comprehensive support for clients.
- Community Outreach: Participate in outreach efforts to identify and engage individuals who may benefit from services.
- Program Compliance: Adhere to all organizational policies, regulations, and procedures, including those related to client privacy and safety.
Qualifications:
- Education: High school diploma or equivalent required. A bachelor’s degree in social work, psychology, or a related field is preferred.
- Skills:

- Strong interpersonal and communication skills, both verbal and written.
- Ability to work collaboratively with a diverse team and clients.
- Good problem-solving and crisis management skills.
- Proficient in using computers and case management software.
- Knowledge of local community resources and social service agencies.
- Other Requirements:
- Ability to maintain confidentiality and handle sensitive information with professionalism.
- Willingness to undergo background checks as required by The Salvation Army and local regulations.
- Flexibility to work evenings or weekends, as needed, based on program requirements.
